right now....Silver Sneakers has LA fitness on the roster, but Silver&Fit does not include LA Fitness.

American Specialty Health Fitness, Inc. (ASH Fitness) is the nation's leading specialty benefits organization for specialty health care and is creating a lifestyle and wellness initiative. We are expanding our focus on fitness and wellness with the continued development of a network of exercise centers, fitness clubs, and personal trainers throughout the nation. ASH Fitness is a benchmark in the creation of benefit plans others have not yet begun to think of offering. ASH Fitness continues to strive to stay one step in front of the others to offer more options, benefits and opportunities to our health plan and employer group clients.
These pages contain everything you need to become a Contracted Facility with ASH Fitness. You will find the Application, Agreement, and instructions for the items you need to return. The Return Checklist will guide you through the materials that need to be filled out and submitted back to ASH Fitness for us to begin the contracting process.

OVERVIEW
American Specialty Health Fitness (ASH Fitness) has a variety of programs for you to choose from. Please see the below descriptions for a short overview of each program.
The Affinity Program is an enhancement to a member's health coverage. The Affinity Program provides members access to ASH Fitness contracted facilities at a minimum 10 percent discount off enrollent fees, and/or monthly dues, or exercise sessions. The Affinity Program is geared toward health plans not yet ready to offer a full benefit program but wish to expose their members to better health through fitness.
The Member Reimbursement Network is an enhancement to a member's health coverage. The Member Reimbursement Network provides members access to ASH Fitness contracted facilities at a minimum 10 percent discount off enrollent fees, and/or monthly dues, or exercise sessions. Eligible members receive reimbursement back from ASH Fitness for enrolling and/or utilizing contracted facilities. Contracted facilities are required to submit monthly utilization to ASH Fitness in order for ASH Fitness to reimburse the member directy. The Member Reimbursement Network is geared toward health plans not yet ready to offer a full benefit program but wish to expose their members to better health through fitness.
The Silver&Fit and Active&Fit Programs allow eligible members to access Contracted Facilities through their health plan coverage. Members have access to any classes or amenities that do not require an extra fee. The Silver&Fit Program is available to Medicare-eligible members. The Active&Fit Program is available to commercial members between the ages of 18 and 64.

We thank you for your interest in being a part of ASH Fitness' network of Contracted Facilities. Below are the materials you need to print, sign and return to ASH Fitness to begin the contracting process. Please print the Return Checklist to assure you have included all necessary documentation required by ASH Fitness to complete the contracting process as quickly as possible.
All fitness facilities wishing to participate must successfully meet the Health and Safety Guidelines and Code of Conduct requirements prior to contract execution.

STEPS AFTER APPLYING - IT'S AS EASY AS 1 2 3
1. JOINING THE FITNESS NETWORK
Facility submits the application to ASH Fitness.
Once the application is received at ASH Fitness it is processed and will take up to 3-4 weeks to become active.
Once facility becomes active a welcome packet is sent to the contracted facility.
Facility location and amenity information is posted on various ASH Fitness Web sites.
2. MEMBER RESPONSIBILITIES AND CONFIRMING MEMBER ELIGIBILITY
Member Responsibilities
The health plan will outreach to all eligible members to notify them of their eligibility to participate in the ASH Group fitness program.
Member contacts ASH Group to enroll in the fitness program and selects a primary facility to utilize the fitness benefit.
Confirming Member Eligibility
When an eligible Silver&Fit or Active&Fit member inquires about the program please proceed with the following:
Ask the member if they have enrolled in the Silver&Fit or Active&Fit program. They can enroll by calling 877.427.4788 option 2 or by accessing the ASH Client's Web site or the applicable ASH Group Web site.
If a member states they have already enrolled refer to the ASH Group's membership roster to verify enrollment. The membership roster is your first point of reference to verify member's enrollment.
If member is not on the membership roster call 877.329.2746 option 1 to verify member enrollment.
When a member enrolls they will choose your facility (members can only choose one facility at a time).
Member will receive an identification card from ASH Group approximately 5-10 days after enrolling.
3. MEMBERS FIRST VISIT, TRACKING AND REPORTING:
Members first visit to the facility
Sign member up on standard contract (zero down)
Fitness Facility will not charge an initiation fee or membership fee.
Issue the member your standard membership card.
Tracking
Upon each visit, the member must present or swipe their membership card. If you have members manually "sign in" please use the Member Sign-In Sheet that will be provided in your welcome packet. You will be required to submit this for compensation.
Reporting
Fitness Facility runs monthly report on member utilization
Information to gather for tracking purposes:
Members name
Members birthdate (month and day)
Members health plan, Silver&Fit or Active&Fit identification number
Members workout dates
Fitness Facility will submit a billing report to ASH Group for compensation after 1 month of utilization.
Reports may be submitted via ASHLink, email, mail or fax.
