I am mostly vegetarian and here is a suggestion that works and is proven at Burger King.

McVeggie meal: three sizes!

Veggie Burger
Fries or onion rings (LMS)
Ice Green tea or regular iced tea - sweetened or unsweetened (LMS)

McDonalds can improve on this idea and pull in $500 million a year with some advertising.
